Pet Visit

We love to have animals come see us, but the following rules must be obeyed.

1. The pet owner must notify the nursing facility prior to the first visit.
    

2. The form below, accompanied by a record of current vaccinations, must be completed and returned before the pet enters the facility the first time.
    

3. Visiting animals must be clean, properly groomed and healthy.  Visitation will not be permitted if an animal is sick, vomiting, flea- or tick-infested or has diarrhea or open sores.
    

4. The pet owner will be responsible for the visiting pet's behavior (i.e. barking, running, jumping, biting).
    

5. The animal must be on a leash at all times and cannot be left unattended unless approved by staff responsible for the facility pet program.
    

6. Visiting animals will not be permitted in any dining areas during meals.
